The emerging consensus among epidemiologists is that the most likely outcome of this outbreak is a new seasonal disease—a fifth “endemic” coronavirus. With the other four, people are not known to develop long-lasting immunity. If this one follows suit, and if the disease continues to be as severe as it is now, “cold and flu season” could become “cold and flu and COVID-19 season.”

At this point, it is not even known how many people are infected. As of Sunday, there have been 35 confirmed cases in the U.S., according to the World Health Organization. But Lipsitch’s “very, very rough” estimate when we spoke a week ago (banking on “multiple assumptions piled on top of each other,” he said) was that 100 or 200 people in the U.S. were infected. That’s all it would take to seed the disease widely. The rate of spread would depend on how contagious the disease is in milder cases. On Friday, Chinese scientists reported in the medical journal JAMA an apparent case of asymptomatic spread of the virus, from a patient with a normal chest CT scan. The researchers concluded with stolid understatement that if this finding is not a bizarre abnormality, “the prevention of COVID-19 infection would prove challenging.”

With so little data, prognosis is difficult. But the concern that this virus is beyond containment—that it will be with us indefinitely—is nowhere more apparent than in the global race to find a vaccine, one of the clearest strategies for saving lives in the years to come.

Over the past month, stock prices of a small pharmaceutical company named Inovio more than doubled. In mid-January, it reportedly discovered a vaccine for the new coronavirus. This claim has been repeated in many news reports, even though it is technically inaccurate. Like other drugs, vaccines require a long testing process to see if they indeed protect people from disease, and do so safely. What this company—and others—has done is copy a bit of the virus’s RNA that one day could prove to work as a vaccine. It’s a promising first step, but to call it a discovery is like announcing a new surgery after sharpening a scalpel.

Inovio is far from the only small biotech company venturing to create a vaccine. Others include Moderna, CureVac, and Novavax. Academic researchers are also on the case, at Imperial College London and other universities, as are federal scientists in several countries, including at the U.S. National Institutes of Health. Anthony Fauci, head of the NIH’s National Institute of Allergy and Infectious Diseases, wrote in JAMA in January that the agency was working at historic speed to find a vaccine. During the SARS outbreak in 2003, researchers moved from obtaining the genomic sequence of the virus and into a phase 1 clinical trial of a vaccine in 20 months. Fauci wrote that his team has since compressed that timeline to just over three months for other viruses, and for the new coronavirus, “they hope to move even faster.”

Except that it wasn’t under control—it still isn’t—and no one knows just how bad it will be. “Even a middle schooler wouldn’t have said that,” Michael Mina, an epidemiology professor at Harvard’s School of Public Health, told me. “Everyone is using caution in how we’re framing what the risk is, primarily because we don’t understand what the risk is at this moment. The last thing anyone would say is, ‘We’re not concerned.’ Everyone is concerned.”

When Trump asked about the next guy in the chain of command over coronavirus, he was told that guy got fired by Trump, too. Trump had literally said, "You're fired!" thinking that the White House is a TV show. Trump may believe casualties in the Trump Show are actually stuntmen trained to fall down and fake death by coronavirus. When the director shouts "Cut!" on the Trump Show, the stuntmen get up again for the next take. If they don't, that's not Trump's problem or responsibility, since he will be golfing or traveling or campaigning.

The White House said President Trump was referring to the Ebola virus — not the deadly coronavirus — when he claimed that “we’re very close to a vaccine.”

President Donald Trump has lots and lots of thoughts about the novel coronavirus. Unfortunately, most of those thoughts are either wrong or factually questionable.

At a press conference during his visit to India on Tuesday, Trump made a number of claims about the virus that were at best hugely optimistic and at worst deeply misleading.

"I think that's a problem that's going to go away," Trump said of the virus.

In that news conference, the President insisted that "they have studied it. They know very much. In fact, we're very close to a vaccine." There's no available evidence that a vaccine is "close," in fact, most infectious disease experts say developing a vaccine for the virus is roughly a year away.

Trump also claimed in the news conference that all the US coronavirus patients "are getting better ... they're all getting better." That followed a tweet very early Tuesday morning in India that read: "The Coronavirus is very much under control in the USA. We are in contact with everyone and all relevant countries. CDC & World Health have been working hard and very smart. Stock Market starting to look very good to me!"

Trump has repeatedly offered rosy predictions about the disease's arc -- most notably pushing the idea that warming temperatures (as winter turns to spring across North America) will reduce its impact much like seasonal flu.

There are mistruths and there MISTRUTHS. And when you are misleading people -- whether intentionally or not -- about a public health emergency, it's very much in that latter category. Because Trump is a known prevaricator and exaggerator, there is a tendency to view all of his mistruths through the there-he-goes-again lens.

It's critically important that this issue of "reinfection" be understood: What kind of immunity is built up (if any)? How often does reinfection occur? Are the symptoms worse that the first infection? Among other things, if the immune response is weak or (worse) counterproductive, then the idea of a vaccine doesn't promise much.

Hmm... I take that back about death rate in Italy. 322 cases, 10 dead, is 3%.

Now. There are a lot of questions about China's reported death rate. One thing that statisticians noted is that it stubbornly stayed at 2.1%. Normally, you would expect to see fluctuation, so the thought is that China is fudging its numbers.

The other problem with "death rate" is that, while it's easy to count the dead, it's a lot more difficult to count the infected, especially if a number of people have mild cases (attributed to normal flu) or are even apparently healthy. If there is a lack of test kits ... or the tests produce false negatives ... then the infection rate wil be undercounted, potentially significantly, which will create an inflated death rate.

I don't know how these things are done in epidemiology-land, but it seems to me that the only way to get a true infection rate is to take a random sampling of the apparently healthy, as well as the apparently sick. Pretty sure that isn't happening anywhere!

Trump’s first budget proposal contained proposed cuts to the CDC that former Director Tom Frieden warned were “unsafe at any level of enactment.”

Congress mercifully didn’t agree to any such cuts, but as recently as February 11 — in the midst of the outbreak — Trump proposed huge cuts to both the CDC and the National Institutes of Health.

Perhaps because his budget officials were in the middle of proposing cuts to disease response, it’s only over this past weekend that they pivoted and started getting ready to ask for the additional money that coping with Covid-19 is clearly going to cost.

Yeah, reading this with fresh eyes I can see how it might be misleading. They would have been more accurate (and less sensational) phrasing it something like ... California tells 7,600 people to 'self-quarantine' because of coronavirus concerns

Still, it does make an important point - there's a LOT of travel between California and China, and so a LOT of potential points of new infection chains here in the US.

"Now is the time for US businesses, hospitals, and communities to begin preparing for the possible spread of #COVID19," it wrote, referring to the name the World Health Organization has given the novel coronavirus. The CDC still doesn't know what that will look like, she added. Community spread could be reasonably mild or very severe.

"We are asking the American public to work with us to prepare in the expectation that this could be bad," she said. "The data over the last week, and the spread in other countries, has certainly raised our level of concern and raised our level of expectation" of community spread, she said.

Americans should also talk to employers about working online and talk to doctor's offices about telehealth, the CDC says.

The viruses can make people sick, usually with a mild to moderate upper respiratory tract illness, similar to a common cold. Coronavirus symptoms include a runny nose, cough, sore throat, possibly a headache and maybe a fever, which can last for a couple of days.

For those with a weakened immune system, the elderly and the very young, there's a chance the virus could cause a lower, and much more serious, respiratory tract illness such as pneumonia or bronchitis.

People may be able to reduce their risk of infection by avoiding those who are sick, avoiding touching their eyes, nose and mouth, and washing hands often with soap and water and for at least 20 seconds.

The coronavirus almost certainly will begin spreading in communities in the United States, and Americans should begin preparations now, officials at the Centers for Disease Control and Prevention said on Tuesday.

I'm just grading myself on the spread of SARS-COV-2. Pretty early on I was extremely critical of WHO, and my opinion was that global spread was inevitable because WHO (Tedros) was far too compromised to do what needed to be done. But I was also far too optimistic that 'somebody' at 'some point' would do 'something' to stop the train I saw coming down the tracks. I was giving the benefit of the doubt, and putting hopeful question marks on what I thought was the near future.

So, if I use past experience with previous corona-virus outbreaks to predict the forward path of this corona-virus "2019-nCoV" outbreak, we have neither drugs nor vaccines to combat it, nor are we likely to have any in the future.

My opinion is that given the information so far - especially the virus's explosive spread far past its origin in mere weeks - officials shouldn't be waiting for proof of pandemic, they should be acting on the potential for one. My personal opinion is that I see no reason why this won't eventually spread globally. For every confirmed case, I think they're at least 5 undiagosed. Those people are infectious, and can go around quietly infecting other people, who infect other people, and so on.
So the start may be small-scale in other countries, but unless either the virus changes, or control measures are adequate, I don't see anything other than a spread of the virus.
But, we'll see.

No one can predict the outcome, with, again IMO, the biggest variable being the number of unidentified cases in the general population. But if 2019-nCoV should get a toehold, the second variable would be if there's brisk and maybe even draconian measures to stop the spread.
I think US authorities will have the necessary data in 2 or 3 weeks about which way this is going.

So, patient zero in this case was spreading coronavirus without a fever for 4-5 days (depending on time of embarking Jan 20 and debarking Jan 25). And either he had a heck of a lot of 'close contact' with 61 other passengers, OR the virus is spread more easily than officials state, and/or newly infected passengers are spreading coronavirus in a very short time.

If there were sufficiently good tests for SARS-COV-2 that were accurate enough, cheap enough, and fast enough - the government could go out and test random people in the population, instead of waiting for sick people to show up in the ER, or at some type of border entry. That would give the government an earlier, better idea of distribution in the particular population they were testing.
