Could you imagine River turning her unprotected back toward the Operative as did Mal after the scene below? I cannot.

If it had been her rather than Mal confronting the Operative, her second shot would have killed (or wounded) the Operative, depending on how she wants the Operative's story to end. Turning her back to the Operative would not be a rational choice. But that was what Mal did. Mal wasn't being "more complex, three-dimensional". He was being flat stupid, tactically. Of course, that ain't exactly plan A.

The tall, thin metal box next to the Operative moved without anybody moving it.
