Inara's Big Secret

I only recently became acquainted with Firefly, and I'm really annoyed that we won't find out Inara's big secret! (Unless more movies are mad,e of ocurse...) I've read comments where people have suggested that she might have some sort of fatal illness, or might be taking some illegal drug, but if she were sick or otherwise unhealthy, wouldn't River have mentioned something to her doctor brother?
I think we can assume that our dear, crazy psychic knows at least some small part of everyone's biggest secret (scenes from Objects in Space seem to indicate that she can pick up on really emotional or intense memories or thoughts almost at random), and she never seemed shy about sharing what's in her head. If Inara's secret was something harmful, River would have piped up about it. Thus, River not only knows something about Inara's big secret, but also knows that it's not evil or harmful, and that Inara has good reason for keeping it secret. (River must also have had some idea of Book's past but, likewise, must have known that blurting out what she knew was unneccessary, since he was trying to do good with his life.)
What if Inara is a smuggler in her own right? Drugs, information, maybe some sort of chemical weapon. Maybe she's helping that underground that Simon mentioned, and maybe she's the connection between high-ranking officials who want to help the effort but keep their names out of it. Maybe the reason she brings out that box when a Reaver attack seems imminent is that she wants to dispose of the item so no one will find it if the ship is ever found. It makes sense that she wouldn't have told Mal & Co. about her sideline at the beginning, since she didn't know them. Later, she might have worried that they were still unreliable, or she just might not have wanted to bring extra trouble on them. Traveling with people who want to stay off the Alliance's radar could only help her keep away from prying eyes, and if the item was found on the ship, she could always blame it's presence on the crew.
