Just the other day I re-watched Stargate (the Kurt Russell movie) for the first time in quite a while and I was surprised to find that from what I remembered the TV show, Stargate SG-1, seemed to follow on quite closely with the core ideas of what was laid down. It obviously added more characters etc but it didn't just ignore the original ones either. It also didn't alter the look of everything too much either. The base, ships, armour all stayed relatively similar with the TV show adding more detail to it all but not changing it to look completely different.
Also I was reading the current LOTR thread and seeing some of the opinions there made me think about my own take on the books/films. Personally I would class the translation as reasonably faithful, but I am well aware of changes that were made and for the most part I can accept these.

So what do you consider to be faithful adaptions between books, movies, TV shows, comics or anything else you care to add?

Depends on what you mean by faithfully. Literally adapting something from one medium to another without changes (or as few as possible) or capturing the spirit and tone of the work?

Something like Sin City is a direct adaptation, which while I liked it, I thought would have been a better movie if it had not been so slavishly faithful to the source.

LA Confidential as a book is nearly unfilmable (at least as a standard length film), but it truly captured the vibe, spirit and themes of the book.

I really liked Watchmen, it stuck pretty close to the comic, and the changes they made all worked well enough.

I also really liked Chronicles of Riddick: Escape from butcher bay. Usually videogame/movie tie-ins are terrible, there seems to be a curse, but that game was great at the time. It was great as a game and gave a nice background for the character. Though in this case it wasn't an adaptation of a story so much as a character/world. It told a different tale within the pre-established world of Pitch Black/Riddick.

That's more due to the speed at which they have to be done, and the licensing fees which eat into their development budget. In the case of the Riddick game, the game studio was owned by Disel, so he was able to give them the room to get it right.
